B.L.D.S.A. Swim Secretaries, Affiliated Club Secretaries and members of the B.L.D.S.A. may obtain a copy of the Associations insurance by contacting the B.L.D.S.A. Safety Officer. Mr Chris Hickling. Tel 01274 830615 e.mail :- chris.hickling@btinternet.com All Swim Secretaries should have an information pack containing advice, policies and risk asessment information. Copies of these and replacement risk assessment forms,Accident forms and Accident books may also be obtained from the Safety Officer.

| Risk Assessments for Championships/events should be completed on the appropriate form and sent to the Safety Officer one month prior to the Championship or event. In consequence, an Accident / Incident Report Form should be despatched to the Safety Officer as soon as possible after the event. |
